Tele-Monitoring with Honeywell HomMed

Thanks to the state-of-the art technology provided through our partnership with Honeywell HomMed, patients on Christian Care Home Health Services are offered the opportunity to have daily check-ups in their homes. Christian Care Home Health is the exclusive provider of the Honeywell HomMed system that collects patients’ vital signs on a daily basis. With the HomMed system, Christian Care Home Health puts hospital-grade monitors in patients’ homes to gather clinical data and track trends in their health status.

Each patient placed on a HomMed monitor has individual parameters established by their personal physician.  Trained technicians are responsible for setup, initial teaching and removal at discharge. Data is collected daily and transmitted to the Christian Care Home offices where a professional nurse reviews each client’s results. Reports are available to family members and physicians by fax and email. Coming soon! With the new HomMed web-portal, family members and physicians will be able to access the client’s results via the web.

How Does It Work?

Honeywell HomMed units are small. They take up about as much space as an alarm clock. A trained technician from Christian Care Home Health will set up the unit in the patient’s home and provide training on its use. The HomMed unit requires a standard electrical outlet and a telephone jack. In those cases where a phone jack is not available, Home Health Services can provide a cell phone card on a limited basis. The Honeywell HomMed unit is programmed with health-concern settings physician recommends.

At the same time each day, a voice prompt will instruct the patient to take vital signs, such as weight, blood pressure, pulse, oxygen saturation, and temperature. The voice guides the patient step-by-step through the process of pushing one of three color coded buttons that enables the System to collect vital signs data.

Following the collection of vital signs, the System will ask a series of questions that the physician has provided. It takes only 3 minutes a day and is easy to do!

Once all the data has been collected, the Honeywell HomMed System transmits the data to a clinician at Christian Care Home Health, who reviews the data. If there are any out-of-the-ordinary readings, the clinician will contact the client. If any vital signs fall outside of the recommended parameters set by the physician, the HomMed unit will signal our clinicians and alert them to the client’s condition.

Christian Care Home Health will provide each client’s physician weekly or monthly graphs of the client’s vital signs, unless otherwise indicated by the physician. These reports allow the physician to see subtle changes in conditions and schedule follow-up office visits as needed.

Why ask for Honeywell HomMed Monitoring System?

Clients are able to avoid multiple illnesses when simple interventions, such as changes in diet or medication, are necessary.

Clients often have fewer emergency room visits, and can stay in their homes, where they are the most comfortable.

Even if the patient is not currently under a physician’s care, the Honeywell HomMed Monitoring System can provide a sense of security, knowing that vital signs are being monitored on a daily basis.

The Honeywell HomMed Monitoring System is not meant to replace, but rather complement visits by the Christian Care Home Health Nurse. The Honeywell HomMed Monitoring System allows a nurse to monitor the patient on a daily basis, even if their actual presence is not on a daily basis. The Honeywell HomMed System is not a substitute or replacement for 911 or other emergency systems.  If an emergency arises, the Honeywell HomMed System cannot signal paramedics, firefighters, or police. www.homMed.com
